Assisted Living communities and memory care communities are numerous, and the task of finding your loved one the ideal new home environment can be stressful. We provide a guided path to senior living by locating communities to meet your needs and expectations. Our complimentary and personalized service eliminates the stress of searching for multiple communities on your own. An experienced senior advisor will locate the right home for your loved one. How are we paid? We are paid a referral fee from the Assisted Living Facility that you choose to move in to.

Introduction

Step 1

We provide an initial overview of our services and answer any questions you may have. We then typically do a phone screening in which we gather information about your loved one’s current living situation, care requirements, desired senior living location, and price range. We will then schedule a time to meet you and your loved one to conduct an in-person assessment.

Assessment

Step 2

During this in-person or virtual meeting, we verify whether or not your loved one is appropriate to reside in an assisted living facility as well as their level of care that they may need. We have a licensed nurse that may evaluate your loved one and answer questions that you may have.

Tours

Step 3

Once Horizon Care Senior Living Advisors identifies communities that are appropriate for your loved one, we schedule you on matching community tours; and answer any questions or concerns that you may have.

Transition

Step 4

Our advisors have a vast understanding of Senior Living admissions requirements. We assist with obtaining proper state-required documentation as well as work with you to ensure a smooth transition into your loved one’s new home. After your loved one settles in, we follow up to ensure that you are happy with your loved one’s new home.
